Fax/mail orders: You may print and fill out this order form.

Phone or e-mail orders: Please include the following information with your order:

• Item number and title
• Quantity
• Billing address and phone number
• Shipping address and phone number, if different
• Any special shipping instructions
• Credit card information, if paying by credit card

Shipping: Shipping and handling charges, and North Carolina sales tax where applicable, will be added to your invoice.

We ship:

• within the United States, via UPS (ground) or Priority/First Class Mail
• to Canada and overseas, via Priority/International First Class Mail
